Western Diagnosis: Acute Gastritis
Medical History: Patient is a 30 year old female student. Average diet. In general runs cold and damp with underlying KID yang def.
Questioning exam: Patient had pain in lower abdomen and left lower quadrant following a night of drinking ETOH (Ethyl Alcohol i.e. beer and alcohol) and greasy foods. Felt burning dull achy pain in LLQ. Loose stools with burning at evacuation.

Western Diagnosis: Hypertension and temporal arteritis
Medical History: Hypertension for 40 year on regular medicine.
Questioning exam: Right sided headache for 1 month, worse at night
Pulse exam: Pulse: regular
Right side headache gets worse when palpated.

Acupuncture Treatment for Fatty Tumors
Chief Complaint: Fatty tumors and dry, corrugated fingernails, occasional leg pain
Medical History: Elderly woman about 70 years old. In generally good health except for her main complaints.
Questioning exam: Visible fatty tumours on upper arms about one to two centimeters in diameter, deformed fingernails, occasional leg pain.
Pulse exam: Generally slippery, especially at the lung and spleen positions. Otherwise thin although not overly weak.

Western Diagnosis: ADD attention deficit disorder
Medical History: 15-year old boy in good overall health, strong constitution.
Questioning exam: Will begin going somewhere and then within 1 minute forget where he was going. Cannot pay attention to schoolwork for longer than 1 minute. Procrastinates with schoolwork constantly. Has good long-term memory. Has no problem memorizing or remembering information.

Chinese Herbs and Acupuncture for PCO
Chief Complaint: would like to get pregnant
Western Diagnosis: Polycystic ovarian syndrome/Infertility
Medical History: History of excessive body hair growth and acne with polycystic ovarian syndrome. Took birth control pill for over 10 years, stopped taking BC 1.5 years ago. Had full fertility workup and did three rounds of Clomid- developed an ovarian cyst. Since cyst development and break from fertility treatments, menstrual bleeding has been heavier and longer, increased cramping, back pain, lots of tissue and clots, dark “tar” color, pink, dark read and brown.

Acupuncture Treatment of Golfer’s Elbow
Chief Complaint: Pain in right forearm near the elbow
Western Diagnosis: Golfer’s elbow
Medical History: Since a month ago, patient has pain in right forearm. It’s localized sharp pain that aggravates when patient tries to lift something and playing golf. 9 out 10 in pain scale and pain has been consistent since the beginning. Irregular diet, plays golf regularly, rides motorcycle occasionally, smokes 10 cig/day.

Medical History: Patient is 50 year old male with 5 year history of digestive disorders. Three years ago he was diagnosed with IBS. Patient has bouts of diarrhea which is foul smelling.
Questioning exam: Patient, feels bloated, gassy, constipated.
Discomfort improves after a bowel movement.

Acupuncture for Pain in the Lower Back and Hips
Chief Complaint: Seasonal Allergies/pain in lower back and hips (SI joints)
Western Diagnosis: Seasonal allergies and Ankylosing Spondilitis
Medical History: Patient is a healthy appearing 28 year old male who has a history of allergies (ragweed, dust) since childhood and more recently diagnosed with Ankylosing Spondilitis 3 years ago after a pain in his toes (middle toe on rt, second on lt). No treatments have affected the seasonal allergies. Patient is currently taking Hydroxychloroquine for the Ankylosing Spondilitis as needed by his rheumatologist. Stiffness in the morning but gets better as day goes along. Nothing remarkable in the diet. Patient is physically active as a father (4 year old) and soon to be again.

Medical History: Patient reported suffering from environmental illness(sensitivity)for approximately 14 years. In this time she had been diagnosed as malabsorbtive and prediabetic. She also suffers from migraines. She eats all organic foods mostly greens. She juices.
