Chief Complaint: Inability to swallow solid food

Medical History: Patient was a four year old girl who had been unable to swallow solid foods for almost two weeks. She had been taken to her M.D. who was unable to find any medical reason for the problem.

Her father brought her to my office and was unable to identify any emotional trauma which might have triggered the problem. Fortunately, she was able to swallow small amounts of liquids, so was not overly dehydrated.

Chief Complaint: 39 years male patient complaining of insomnia, not effective sleeping, awakening tired in the morning, dizziness, fatigue, pensiveness, loss of appetite, feeling of heaviness in the epigastric region, nausea but not vomiting.

Medical History: The patient presented has no idea as to the cause of his condition. It exists for long ago with a slow onset but the symptoms were aggravated from one month, especially the mental tiredness, fatigue and insomnia.

He is not smoking, drinking alcohol only on a special occasion, with poor appetite and impaired sleeping. He has a stressful job.Suffer from dizziness.There is nothing abnormal with the defecation and passing urine. He is running each and every weekend.

Medical History: This 20-year-old female(height of 5.2 feet and weight of 105 pounds) came to Acutherapy & Herbal Clinic with a main complaint of dull periodic pain in the lower abdomen prior to and during menstrual period. She began suffering from this problem since age of 13 when she had the first period. At first, she took some pain killer when the pain happened, and then, she went to MD for check up. There were no sign of any organic diseases in the pelvic cavity. At last, she decided to try the alternative medicine for this problem.

Medical History: 41 yo male patient was in MVA 11/20/2000. Pt. states he was stationary at stop sign and was struck from rear by trash truck moving approximately 8mph. Pt. denies striking any object inside of car. Pt. states he was wearing seat belt.

Questioning exam: Pt denies dizziness, blurred vision, nausea. Pt also denies and loss of movement on limbs or any burning or numbness sensations. Pt. states neck is very sore and that he can barely move it.
