Chief Complaint: Post Shingles Neuralgia
Western Diagnosis: Uveitis
Medical History: 93 y.o., female, complaining of paresthesia around left eye with inflammation in left eye following a Herpes Zoster attack which manifested on the upper left quadrant of the face a few months ago. Western treatments with acyclovir, prednisone (eye drops) and ciproflaxin (eye drops) were ineffective for 2 months. Ets 2-3 meals with adequate caloric intake and digestion is ok with no diarrhea. Sleeps a lot- 10+ hours per day. Good disposition.

Herbs and Acupuncture for Diabetic Neuropathy
Chief Complaint: Lower extremity numbness, tingling, and pain for 6 years
Western Diagnosis: Peripheral Diabetic Neuropathy
Medical History: 6 years ago the patient was diagnosed with type 2 diabetes; concurrent with his diagnosis he began to experience signs of peripheral neuropathy in his lower limbs which include numbness, tingling and pain in the ankles and feet. The pain pattern is as follows: the toes are numb, the top of the sole and the sole of the feet are numb, and a numb pain and feeling of thickness with pain are felt around the ankle joint. There is also heaviness of the foot. The patient also reports a dull pain in the knees. The pain is on the yin side of the legs and the UB channel. The pain is aggravated by walking and is worse in cold wet weather. The pain is reported as a 6/10. Alleviating factors are warmth and massage.

Chinese Medicine for Urinary Retention and BPH
Chief Complaint: Urinary retention
Western Diagnosis: Urinary retention due to benign prostatic hypertrophy
Medical History: 68 year old male experiencing increased frequency of urination, especially at night. After urinating, he felt some of the urine remained. Over time, increasing effort was needed to urinate and the urine stream became thin. Enuresis occurred occasionally. Recently, after becoming tired, he was unable to pass urine, resulting in lower abdominal discomfort. The abdominal distention was relieved by catheterization combined with drugs and Chinese herbal medicine. Two days later, voluntary urination was still impossible, and catheterization was repeated. Surgery was recommended, but the patient refused, opting instead for acupuncture.

Medical History: Client has recently had back surgery, is slowly coming off oxycodone, presents with severe depression. Low appetite, little pain at this point. NO exercise as it is not appropriate at this time. Wears a back brace.
Questioning exam: severe depression, suicidal ideation, recent back surgery, some pain
Pulse exam: tight, wiry, heart pulse superficial, slightly rapid.

History: 25 year old female. Menarche at 14 years old. No history of significant illnesses or trauma.
Symptoms: Delayed menstruation since menarch. Menstrual cycle between 40-60 days. Day 34 of cycle. Scanty, pale menstrual blood. Cramps, occasional clots, low back pain with onset of menses. Headaches and fatigue after menses.

Medical History: Female, 67y. She has chronic diarrhea for 10 years, which was diagnosed as IBS. Diarrhea 4 to 5 times everyday, which refrains her from going out, loose stools, no obvious abdominal pain before diarrhea, poor sleep, dry mouth.
Questioning exam: Tenderness around umbilicus. She is stressful, but denied diarrhea related to emotional disturbance.

Acupuncture and Herbs for Migraine Headaches
Chief Complaint: Migraines
Medical History: Patient M is a 32 year old Italian Male who resides in Los Angeles, California and works for a moving company. He presented in my office two years ago with complaints of Migraine headaches that he has had since he was a child. He has tried taking medication for his headaches, but they have not helped.
Since Mr. M was about twelve years old he has experienced migraine headaches about every other week. These headaches last for about one day and range from moderate to severe. Unless his headaches are severe the patient continues to go to work and perform his daily activities. He reports that his headaches feel like pressure inside his head and can sometimes feel like stabbing pain. His usually experiences them on only one side, but that sometimes it is the left and sometimes it is on the right, but never both sides in one episode.

Western Diagnosis: Lumbar Spinal Stenosis
Medical History: Patient is a 82 year old male. He has had back pain for over 10 years. Relevant symptoms include achy, radiating sensation down the anterior right thigh and leg. Numbness and tingling in both feet. Major pain is at the right sacral area at the PSIS. It is tender to the touch. He has had back surgery, laminectomy of the L2,3 and 4 vertebrea. Left knee replacement 7 years ago and right hip replacement 15 years ago. he complains of no pain any where else in the body.
