Chief Complaint: Muscular pain/ achiness
Western Diagnosis: Fibromyalgia
Medical History: Patient: female, 42 years old
Since February 2000, patient had experienced muscular pain. In April 2000 she was diagnosed with Lyme’s dx and put on antibiotics. After no relief, a spinal tap and Tender Point Assessment test were performed. She was diagnosed with fibromyalgia in the summer of 2000. Significant medical history includes saline breast implants in 1983, gall bladder was removed in 1984 and in 1993 the patient was diagnosed with severe allergies and asthma. Medication at the time of visit included Arthrotec and Maxalt.

Chief Complaint: body aches all over
Western Diagnosis: fibromyalgia
Medical History: the patient is an electrician who loves spicy food, exercises not at all. Being an electrician in Maine working in very hot and humid environments in the summer and very cold and windy environments in the winter
Questioning exam: Pain migrates, is better when cold, the pain is dull achy in nature
Pulse exam: pulse is slippery, slightly wiry, large joints appear swollen and red and are hot to the touch

Chief Complaint: fibromyalgia and chronic fatigue
Western Diagnosis: fibromyalgia Jan 2001
Medical History: A 50 year old female, health care professional and PhD candidate, sought Chinese medicine for the first time on May 11, 2001, with symptoms of fibromyalgia for one and a half years prior to formal diagnosis in January 2001.
Patient’s physical activity had been limited, since exercise aggravated her symptoms. Symptoms were also aggravated by stress and inactivity. A physician prescribed Prozac, 10 mg on odd days, 20 mg on even days, Premarin 1.25 mg daily, and glucosamine chondroitin, dosage unspecified. The patient also took a daily multivitamin and calcium supplement. There was mild improvement in her condition on this regimen, but she was advised that she would “just have to live with pain.”

Medical History: Patient reported suffering from environmental illness(sensitivity)for approximately 14 years. In this time she had been diagnosed as malabsorbtive and prediabetic. She also suffers from migraines. She eats all organic foods mostly greens. She juices.

Chief Complaint: Body Pain
Western Diagnosis: Fibromyalgia
Medical History: Patient is a 36 year old female who has a chief complaint of fibromyalgia. She has been experiencing pain in her knees, hips and low back for the past 10 years. This pain began around the same time that she had surgery for bilateral epicondylitis. This patient has experienced a lot of stress in her life related to work and a divorce several years ago. She is only able to exercise occasionally because of pain limitations. She has smoked 1 pack of cigarettes a day for the past 20 years. She does not consume alcohol or caffeine. She has a history of fibroids and gallstones, and her gallbladder was removed 5 years ago. She was also diagnosed with Irritable Bowel Syndrome 5 years ago.

Taking oxycodone for pain, and ambien and trazadone for sleep

Chief Complaint: fatigue, muscle pain, anxiety, depression, insomnia
Western Diagnosis: fibromyalgia
Medical History: Patient is a 45 year-old female, complains of fatigue, lack of energy and muscular pain and joint pain that travel from areas to areas, light headache.
Questioning exam: Patient feel anxious in crowded area, have problem to digest food, have acid reflex and indigestion problem. Patient depresses since her last daughter had married and moved away from home.
