Chief Complaint: Severe Rash

Western Diagnosis: Poison Wood Rash

Medical History: 57 year old male in good heath with moderately good diet, Plays basketball three times a week for exercise. Past history of fungal type rashes and respiratory infections.

Questioning exam: Patient was hiking on vacation in the Bahamas and brushed against a poison wood tree coming in contact with a black sappy substance. Unaware of what it was, patient tried to remove the sap from his arm. This sap then got under his finger nails and was unknowingly spread to other parts of the body. This sap later caused a severe red, raised rash that oozed and crusted over with bright red margins. Since patient was long distance and had few resources, he was advised to use tea bags on lesions, drink mint tea, and eat cooling foods until return home for treatment.

Medical History: 37 year old male presents with acute low back pain occurring for two weeks after turning to the left while squatting, and simultaneously holding a tire/wheel assembly, during an emergency tire change. Patient is a taxi cab owner/driver, sedentary, and a regular smoker.

Questioning exam: Patient has desire to sleep after eating, a bitter taste in mouth and experiences acid reflux. He drinks 8 cups of coffee daily, in addition to sodas. He sleeps in an uncomfortable bed 3-5 hours a night, has trouble falling asleep, and wakes up at night due to light, restless sleep and occasional disturbing dreams. Patient also reports an energy level of 5/10, low libido, a left knee that has pain in response to changes in weather, and a history of migraine headaches for 4 months.

Questioning exam: Right eye pressure and pain for one week. Pressure behind the eye. Stress and worry about money and health issues. Bp was normal on examination. Difficulty falling asleep, prone to anger and feeling overwhelmed.

Western Diagnosis: Hives and Acid reflex associated with stress

Medical History: Patient is a 40 year old female, who generally is in good health. She has had hives since childhood, hives comes and goes. Anti-histamine medication or prednisone did not help in the past. She used to take patent herbs for a few days, the rashes would be gone for a couple of months. Wherever she did not get enough sleep or stressed out, the rashes showed up again. Also each time she got pregnant, she tends to get hives. She never had heart burn before, but sometimes has bloating and indigestion when she eats greasy food .
